Select custom amount. Add $125 for a single, $250 for two, $375 for three, or $500 for four participants or the amount for one of the sponsorship levels.
Check the box “Write us a Comment.” Add the NBH golf tournament and list team members or sponsor names and package levels in the comment.
Click next, Select Donation on Behalf of a company, and type in NorthStar Behavioral Health.
Click Next, and enter card payment.
You will receive a receipt via email that you should save for verification of payment, and tax purposes and forward to info@northstarbehavioralhealthmn.com
